Umzumbe Local Municipality - Incompetence of a ward counselor

At home , we had a funeral and the counselor who attended was supposed to fill in the 1680 form so that as a family we would be able to apply for a death certificate from the Department of Home Affairs. The counselor didn’t fill the form but just wrote a letter confirming his presence in the funeral. All these years we are trying to apply for the death certificate of our late mother but we don’t get help because the Department of Home Affairs need the 1680 form . We have tried countless times to engage with the counselor but he is not taking the matter seriously.
